Standards Vary

That one is easily the worst I have seen although we used a CS earlier where it was under a heavy manhole cover and water to rinse came from a water but that was empty. I had to use the watering can I use to fill the toilet to get water to rinse out.
 
A site i used in the past, added a few courses of bricks to raise the manhole for his septic tank. If you were lucky, you didn't drop your toilet cassette in the hole - or yourself.

One thing to watch out for - A friend once dropped the toilet cassette screw cap down the hole.never to get it back.He had a problem getting a replacement.Ever since then I've always kept the screw cap away from the drain hole.
