Touring Europe with a dog - planning

Our dog has a valid PETS passport and she had an anti-rabies vaccination in March 2008 - I am also aware of the procedure for bringing her back into the UK at the end of the trip. What I am unclear about are the entry requirements between EU/Non EU countries that we might be visiting. Do some countries require annual rabies boosters? Do they require tick and worm treatments similar to re-entry to the UK? The countries we may visit are France, Belgium, Germany, Switzerland, Austria, Czech Republic,Italy, Slovenia, Croatia, Luxembourg, Denmark, Norway, Sweden.
