Has anyone heard of a tow ball breaking while towing and what was the cause? How do we prevent this from happenning

I have never heard of such an incident. The homologation process that towbar manufacturers must go through to obtain legal approval is intended to prevent the worst from happening. However, it is possible that some manufacturers of cheap towbars cut corners regarding corrosion prevention, so it's worth checking if there is unusual amount of rust on the towbar or obvious deep cracks on the surface.
